SUMMARY FLOW CHART

During the spring or summer:

Read handbook on HEARTLINK Program
Request application form
Complete and return application form with registration fee
Make appointment for assessment
Meet with principal of Damascus Christian School, Home School Liaison, and possibly the grade-level teacher.

Beginning of school year:

Prepare yearly instructional objectives
Beginning of each month:
Prepare monthly instructional goals/objectives
Beginning of each week:
Prepare daily lesson plans (see sample)

Mid-year:

Fill out mid-year evaluation
Year-end:
Go over year -end evaluation with teacher and Home School Liaison
